Clinic provides care through Air Force Medical Home

The 341st Medical Group has a mission to enable nuclear surety and optimize the health, wellness and readiness of patients through promoting a culture of trusted care.
One way the base clinic accomplishes this is through Air Force Medical Home.
Medical home allows patients to be assigned to a healthcare team who work directly with them and their family. The healthcare team includes a physician and nurse practitioner or physician assistant who partner with the patient to provide prevention, wellness, acute and chronic care.
“AFMH is about taking care of all of our patient's needs, not just seeing them for their regular checkups but also to help them live longer and healthier lives,” said Lt. Col. Deborah Davidson, 341st Medical Operations Squadron commander. “AFMH integrates our patients into the healthcare team and offers aggressive prevention and personalized treatments and interventions. It means building an organization that makes fewer mistakes, establishes a trusting relationship and leverages new techniques and technologies.”
The medical home staff coordinates all aspects of care to include providing referrals, obtaining results and consulting with other specialty providers.
The need for additional team members such as case managers, disease managers, healthcare educators, behavioral health consultants or pharmacists can be added to the team to ensure an all-inclusive plan of care is provided.
The medical home staff strives to provide the highest quality and safest care possible by using current evidence-based medicine and a focus on measuring staff performance.
"We strive to build rapport and a caring relationship with our patient population,” said Maj. Ryan Seymour, 341st MDOS physician assistant. “By doing this, the patients feel more at ease and comfortable discussing their medical needs with the same team of people at each visit."
In addition to medical home, the clinic offers a secure messaging patient portal which allows patients to book appointments, activate refills, access labs and radiology results, and send and receive secure messages. Patients can also upload attachments and check on referrals.
In addition, patients can utilize the virtual visit, a phone consultation used for follow-ups and medication refills which saves patients’ time as opposed to an appointment.
